I am unable to request for a cheque book through Internet Banking

This could be because:
a. Account would not have been 6 months since opening 
b. More than 40 unused cheque leaves remaining in the current cheque book
c. Account category does not have cheque book facility
d. Contact details changed in the last 1 month
