You will need to fill the Customer Request Form and submit a copy of valid address proof at the branch. You will require a proof of your communication address by way of Aadhaar or other Officially Valid Documents (OVD) listed below:
PAN Card
Valid Passport
Election Card
Photo Identity issued by Government / Defence services / Public Sector Undertaking
Valid Driving Licence with photograph
Photo Credit Card (Unexpired)
Employee ID card
Letter/card issued by UIDAI containing the details of name, photograph, address and Aadhaar number
Bank Passbook with photograph issued by nationalized banks. This passbook should have transactions for a period not more than 3 months old from the application date with a minimum of three customer-induced transactions. A cross-stamp or signature of the authorised signatory of the bank is required on the photograph.
